ALBERT EINSTEIN HAND SURGERY
FELLOWSHIP AT BETH ISRAEL MEDICAL CENTER

GOAL: A thorough education in the principles and practice of hand surgery and upper extremity surgery to enable the fellow to develop a level of proficiency commensurate with the ability to function as a highly skilled physician and with the capacity to enhance, academically and clinically, the quality of hand surgery.

DESCRIPTION: The fellowship provides an in-depth training in surgery of the hand and upper-extremity. The fellow is exposed to large clinical practice with diverse aspects of patient management. Over 1,600 surgeries and 10,000 out patient visits occur annually. The variety of patients seen includes congenital anomalies, fractures of the hand and wrist, joint degeneration, vascular conditions, microsurgery, elbow reconstruction, sports related injuries, and a multitude of soft tissue trauma. Other key components of the fellowship are the extensive exposure to the amateur, Olympic and professional sport injuries of the hand and wrist. There is a wide exposure to multifarious types of arthritis, including scleroderma with its unique vascular and wound problems. There is constant exposure to on-premises hand rehabilitation techniques, with an opportunity to design and fabricate orthotics under the training of our certified hand therapist. A microsurgical laboratory is available, with a dedicated one week course, for enhanced training in increasingly sophisticated micro-techniques. Additional Microvascular exposure is gained through our close association with Montefiore Medical Center replant/revascularization team.  Additionally, the Hand Center has an on on-site procedure room available for manipulations, closed reductions/pinning, and simple open reductions.

Our fellow will evaluate and manage a large volume of diverse hand problems in his/her year of training. Direct responsibility for resident teaching, hand journal club, and monthly conferences are responsibilities of the fellow. Preoperative assessment, evaluation of special studies, intra-operative management and postoperative care are major components of the surgical training aspects of the fellowship.  At our weekly teaching conferences we will discuss upcoming cases to improve the fellow’s understanding of the indications for surgery, as well as the surgical procedure. The fellow will also direct a weekly academic conference devoted to core curriculum hand topics, present and discuss articles at monthly journal clubs, as well as attend and contribute to the monthly Beth Israel orthopaedic grand rounds. An additional didactic session includes radiology conference, reviewing MRI, MRA, Cine Studies and CT Scans of problem cases.

During the course of the academic year, the fellow is expected to complete at least one clinical research project and present this original work at the New York Society for Surgery of the Hand, Resident & Fellow Conference, with submission to the annual meeting of the ASSH. On call responsibilities are generally limited to home-call and are divided amongst the fellow and three dedicated hand physician assistants.
